What it is and what it is used for

Zolmitriptan Orodispersible Tablets contains zolmitriptan and belongs to a group of medicines called triptans.

Zolmitriptan Orodispersible Tablets is used to treat migraine headache.

• Migraine symptoms may be caused by the widening of blood vessels in the head. Zolmitriptan Orodispersible Tablets is thought to reduce the widening of these blood vessels. This helps to take away the headache and other symptoms of a migraine attack, such as feeling or being sick (nausea or vomiting) and being sensitive to light and sound.
• Zolmitriptan Orodispersible Tablets works only when a migraine attack has started. Do not use it to prevent a migraine attack.

What you need to know before you take it

Do not take Zolmitriptan Orodispersible Tablets: • if you are allergic to zolmitriptan or any of the other ingredients of this medicine (listed in section 6)
• if you have high blood pressure
• if you have ever had heart problems, including a heart attack, angina (chest pain caused by exercise or effort), Prinzmetal's angina (chest pain which happens at rest) or have experienced heart related symptoms such as shortness of breath or pressure over the chest
• if you have had a stroke or short-lasting symptoms similar to stroke (transient ischaemic attack or TIA)
• if you have severe kidney problems
• if you are at the same time taking some other medicines for migraine (e.g. ergotamine or ergot-type medicines like dihydroergotamine and methysergide) or other triptan medicines for migraine. See section on 'Other medicines and Zolmitriptan Orodispersible Tablets' for further information.
If you are not sure if any of these apply to you, talk to your doctor or pharmacist

Warnings and precautions Talk to your doctor before taking Zolmitriptan Orodispersible Tablets. Let your doctor know if:

• you are at risk of getting ischaemic heart disease (poor blood flow in the arteries of the heart). Your risk is greater if you smoke, have high blood pressure, high levels of cholesterol, diabetes or if anyone in your family has ischeamic heart disease
• you have been told that you have Wolff-Parkinson-White Syndrome (a type of abnormal heart beat)
• you have ever had liver problems
• you have headaches which are not like your usual migraine headache
• you are taking any other medicines for treatment of depression (see section below for 'Other medicines and Zolmitriptan Orodispersable Tablets' for further information).
If you go into hospital tell the medical staff you are taking Zolmitriptan Orodispersible Tablets.

Zolmitriptan Orodispersible Tablets is not recommended for people aged under 18 years, or over 65.

As with other migraine treatments, taking too much Zolmitriptan Orodispersible Tablets can cause daily headaches or can make your migraine headaches worse. Ask your doctor if you think that this is the case for you. You may need to stop taking Zolmitriptan Orodispersible Tablets to correct the problem.

Other medicines and Zolmitriptan Orodispersable Tablets Tell your doctor or pharmacist if you are taking, have recently taken any or might take any other medicines. This include herbal medicines and medicines you buy without prescription.

In particular, tell your doctor if you are taking any of the following medicines:

Medicines for migraine

• If you take triptans other than Zolmitriptan Orodispersible Tablets, leave 24 hours before taking Zolmitriptan Orodispersible Tablets.
• After taking Zolmitriptan Orodispersible Tablets, leave 24 hours before taking triptans other than Zolmitriptan Orodispersible Tablets.
• If you take medicines containing ergotamine or ergot-type medicines (such as dihydroergotamine or methysergide), leave 24 hours before taking Zolmitriptan Orodispersible Tablets.
• After taking Zolmitriptan Orodispersible Tablets, leave 6 hours before taking ergotamine or ergot-type medicines.
Medicines for depression

• moclobemide or fluvoxamine.
• medicines called SSRIs (selective serotonin reuptake inhibitors).
• medicines called SNRIs (serotonin norepinephrine reuptake inhibitors) such as venlafaxine, duloxetine.
Serotonin syndrome is a rare, life-threatening condition that has been reported in some patients who took Zolmitriptan Orodispersible Tablets in combination with so called serotoninergic medicines (e.g. certain medicines for the treatment of depression). Signs of serotonin syndrome may be for example, agitation, tremor, restlessness, fever, excessive sweating, twitching, muscle rigidity uncoordinated movement of limbs or eyes and uncontrollable jerking of muscles. Your doctor may advise you on this.

Other medicines

• cimetidine (for indigestion or stomach ulcers)
• a quinolone antibiotic (such as ciprofloxacin)
If you are using herbal remedies containing St John's Wort ( Hypericum perforatum ), side effects of Zolmitriptan Orodispersible Tablets may be more likely to happen..

Zolmitriptan Orodispersible Tablets with food and drink You can take Zolmitriptan Orodispersible Tablets with or without food. It does not affect the way that Zolmitriptan Orodispersible Tablets works.

Pregnancy, breast-feeding and fertility If you are pregnant or breast-feeding think you may be pregnant or are planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.

• It is not known if taking Zolmitriptan Orodispersible Tablets during pregnancy is harmful. Before taking Zolmitriptan Orodispersible Tablets, tell your doctor if you are pregnant or trying to become pregnant.
• Do not breast-feedwithin 24 hours taking zolmitriptan.
Driving and using machines • During a migraine attack your reactions may be slower than usual. Bear this in mind when drive or use any tools or machines.
• Zolmitriptan Orodispersible Tablets is unlikely to affect driving or using tools or machines. However, it is best to wait to see how Zolmitriptan Orodispersible Tablets affects you before you try these activities.
Zolmitriptan Orodispersible Tablets contains Aspartame (E951) Zolmitriptan 2.5 mg Orodispersible Tablets contains 2 mg aspartame (E951) in each orodispersible tablets.

Zolmitriptan 5 mg Orodispersible Tablets contains 4 mg aspartame (E951) in each orodispersible tablets. Aspartame is a source of phenylalanine. It may be harmful if you have phenylketonuria (PKU), a rare genetic disorder in which phenylalanine builds up because the body cannot remove it properly.

How to take it

Always take this medicine exactly as your doctor has told you. Check with your doctor or pharmacist if you are not sure.

The recommended dose is 2.5 mg or 5 mg. Zolmitriptan Orodispersible Tablets is used to treat migraine attacks. Take Zolmitriptan Orodispersible Tablets as soon as possible after your migraine headache has started. You can also take it once an attack is underway.

If the symptoms of migraine return within 24 hours, you may take a second dose of zolmitriptan. The second dose should not be taken within 2 hours of your first dose.

If the tablets did not relieve your migraine, tell your doctor. Your doctor may raise the dose to 5 mg or change your treatment.

Do not use more than two doses in one day. The maximum daily dose is 10 mg zolmitriptan.

Method of administration

Oral Use.

Peel the blister pack open as shown on the foil. Do not push the tablet through the foil.

Place the tablet on your tongue, where it will dissolve and be swallowed with the saliva. You do not have to take a drink of water in order to swallow your tablet.

If you take more Zolmitriptan Orodispersible Tablets than you should If you have taken more Zolmitriptan Orodispersible Tablets than prescribed by your doctor, tell your doctor or go to the nearest hospital straight away. Take the Zolmitriptan Orodispersible Tablets medicine with you.

If you forget to take Zolmitriptan Orodispersible Tablets Do not take a double dose to make up for a forgotten tablet.

If you have any further questions on the use of this product, ask your doctor or pharmacist.

Possible side effects

Like all medicines, this medicine can cause side effects, although not everybody gets them. Some of the symptoms below could be part of the migraine attack itself.

Stop taking Zolmitriptan Orodispersible Tablets and contact your doctor straight away if you experience any of the following side effects: Rare (may affect up to 1 in 1,000 people)

• Allergic/hypersensitivity reactions including lumpy rash (hives), swelling of the face, lips, mouth, tongue or throat, difficulty swallowing or breathing difficulties.
Very rare (may affect up to 1 in 10,000 people)

• Angina (pain in the chest, often brought on by exercise), heart attack or spasm of the blood vessels of the heart. You may notice chest pain or shortness of breath.
• Spasm of the blood vessels of the gut, which can cause damage to your gut. You may notice stomach pain or bloody diarrhoea.
Other side effects that may occur: Common (may affect up to 1 in 10 people):

• Abnormal sensations such as tingling in your fingers and toes or skin that is sensitive to touch.
• Feeling sleepy, dizzy or warm
• Headache
• Uneven heart beat
• Feeling sick, Vomiting
• Stomach pain
• Dry mouth
• Muscle weakness or muscle pain
• Feeling weak
• Heaviness, tightness, pain or pressure in throat, neck, arms and legs, or chest
• Difficulty in swallowing
Uncommon (may affect up to 1 in 100 people):

• Very fast heart beat.
• Slightly higher blood pressure.
• Increase in the amount of water you pass (urine) or in how often you need to pass water.

How to store it

Keep this medicine out of the sight and reach of children.

Do not use this mediicne after the expiry date which is stated on the carton after EXP: The expiry date refers to the last day of that month.

• This medicinal product does not require any special storage conditions.
• Do not throw away any medicines via wastewater or household waste. Ask your pharmacist how to throw away medicines you no longer use. These measures will help to protect the environment.

The other ingredients are Mannitol (E421), Cellulose microcrystalline, Crospovidone Type A, Aspartame (E951), Silica colloidal anhydrous, Talc, Magnesium Stearate, Peppermint Flavour.

4.1. Therapeutic indications

Zolmitriptan is indicated in adults aged 18 years and older for acute treatment of migraine headache with or without aura.

4.2. Posology and method of administration

Posology

The recommended dose of zolmitriptan to treat a migraine attack is 2.5 mg. It is advisable that Zolmitriptan Orodispersible Tablets is taken as early as possible after the onset of migraine headache but it is also effective if taken at a later stage.

If symptoms of migraine should recur within 24 hours following an initial response, a second dose may be taken. If a second dose is required, it should not be taken within 2 hours of the initial dose. If a patient does not respond to the first dose, it is unlikely that a second dose will be of benefit in the same attack.

If a patient does not achieve satisfactory relief with 2.5 mg doses, for subsequent attacks 5 mg doses of zolmitriptan could be considered. The total daily intake should not exceed 10 mg. Not more than 2 doses of zolmitriptan should be taken in any 24-hour period.

Zolmitriptan is not indicated for prophylaxis of migraine.

Paediatric population

Use in Children (under 12 years of age)

The safety and efficacy of zolmitriptan tablets in children aged birth to < 12 years have not been established. No data are available. Use of zolmitriptan in children is therefore not recommended.

Adolescents (12 - 17 years of age)

The efficacy of zolmitriptan tablets in children aged 12 to 17 years have not been established. Currently available data are described in section 5.1 but no recommendation on a posology can be made. Use of zolmitriptan in adolescents is therefore not recommended.

Special populations

Use in patients aged over 65 years

The safety and efficacy of zolmitriptan in individuals aged over 65 years have not been evaluated. Use of Zolmitriptan in the elderly is therefore not recommended.

Patients with hepatic impairment

The metabolism of zolmitriptan is reduced in patients with hepatic impairment (see section 5.2). For patients with moderate or severe hepatic impairment, a maximum dose of 5 mg in 24 hours is recommended. However, no dose adjustment is required for patients with mild hepatic impairment.

Patients with renal impairment

No dosage adjustment required in patients with a creatinine clearance of more than 15 ml/min. (see section 4.3 and section 5.2).

Interactions requiring dose adjustment (see section 4.5).

For patients taking MAO-A inhibitors, a maximum dose of 5 mg in 24 hours is recommended.

A maximum dose of 5 mg zolmitriptan in 24 hours is recommended in patients taking cimetidine.

A maximum dose of 5 mg zolmitriptan in 24 hours is recommended in patients taking specific inhibitors of CYP 1A2 such as fluvoxamine and the quinolones (eg ciprofloxacin).

Method of administration

The blister pack should be peeled open as shown on the foil (tablets should not be pushed through the foil). The zolmitriptan orodispersible tablet should be placed on the tongue, where it will dissolve and be swallowed with the saliva.

The tablet need not be taken with liquid; the tablet dissolves on the tongue and is swallowed with saliva. This formulation can be used in situations in which liquids are not available, or to avoid the nausea and vomiting that may accompany the ingestion of tablets with liquids. However, a delay in the absorption of zolmitriptan from zolmitriptan can occur which may delay onset of action.

4.3. Contraindications

Hypersensitivity to the active substance or to any of the excipients listed in section 6.1.

Moderate or severe hypertension and mild uncontrolled hypertension.

This class of compounds (5HT1B/1D receptor agonists), has been associated with coronary vasospasm, as a result, patients with ischaemic heart disease were excluded from clinical trials. Therefore, zolmitriptan should not be given to patients who have had myocardial infarction or have ischaemic heart disease, coronary vasospasm (Prinzmetal's angina), peripheral vascular disease or patients who have symptoms or signs consistent with ischaemic heart disease.

Concurrent administration of ergotamine, ergotamine derivatives (including methysergide), sumatriptan, naratriptan and other 5HT1B/1D receptor agonists with zolmitriptan is contraindicated (see section 4.5).

Zolmitriptan should not be administered to patients with a history of cerebrovascular accident (CVA) or transient ischaemic attack (TIA)

Zolmitriptan is contraindicated in patients with a creatinine clearance of less than 15 ml/min.

4.4. Special warnings and precautions for use

Zolmitriptan should only be used where a clear diagnosis of migraine has been established. As with other acute migraine therapies, before treating headaches in patients not previously diagnosed as migraineurs, and in migraineurs who present with atypical symptoms, care should be taken to exclude other potentially serious neurological conditions. Zolmitriptan is not indicated for use in hemiplegic, basilar or ophthalmoplegic migraine. Stroke and other cerebrovascular events have been reported in patients treated with 5HT1B/1D agonists. It should be noted that migraineurs may be at risk of certain cerebrovascular events.

Zolmitriptan should not be given to patients with symptomatic Wolff-Parkinson-White syndrome or arrhythmias associated with other cardiac accessory conduction pathways.

In very rare cases, as with other 5HT1B/1D agonists, coronary vasospasm, angina pectoris and myocardial infarction have been reported. Zolmitriptan should not be given to patients with risk factors for ischaemic heart disease (e.g. smoking, hypertension, hyperlipidaemia, diabetes mellitus, heredity) without prior cardiovascular evaluation (see section 4.3). Special consideration should be given to postmenopausal women and males over 40 with these risk factors. These evaluations, however, may not identify every patient who has cardiac disease, and in very rare cases, serious cardiac events have occurred in patients without underlying cardiovascular disease.

As with other 5HT1B/1D receptor agonists, heaviness, pressure or tightness over the precordium (see section 4.8) have been reported after the administration of zolmitriptan. If chest pain or symptoms consistent with ischaemic heart disease occur, no further doses of zolmitriptan should be taken until after appropriate medical evaluation has been carried out.

As with other 5HT1B/1D agonists transient increases in systemic blood pressure have been reported in patients with and without a history of hypertension. Very rarely these increases in blood pressure have been associated with significant clinical events. The dose recommendation for zolmitriptan should not be exceeded.

Serotonin Syndrome has been reported with combined use of triptans, and serotonergic drugs, such as selective serotonin reuptake inhibitors (SSRIs) and serotonin norepinephrine reuptake inhibitors (SNRIs). Serotonin syndrome is a potentially life-threatening condition and diagnosis is likely when (in presence of a serotonergic agent) one of the following is observed:

• Spontaneous clonus

• Inducible or ocular clonus with agitation or diaphoresis,

• Tremor and hyperreflexia

• Hypertonia and body temperature >38°C and inducible or ocular clonus.

Careful observation of the patient is advised if concomitant treatment with ZOMIG and an SSRI or SNRI is necessary, particularly during treatment initiation and dosage increases (see Section 4.5).

Withdrawal of the serotonergic drugs usually brings about a rapid improvement. Treatment depends on the type and severity of the symptoms.

Prolonged use of any type of painkiller for headaches can make them worse. If this situation is experienced or suspected, medical advice should be obtained and treatment should be discontinued. The diagnosis of medication overuse headache should be suspected in patients who have frequent or daily headaches despite (or because of) the regular use of headache medications.

Zolmitriptan 2.5 mg Orodispersible Tablets contains 2 mg aspartame in each 2.5 mg orodispersible tablet. Zolmitriptan 5 mg orodispersible tablets contains 4 mg aspartame in each 5 mg orodispersible tablet.

Aspartame is a source of phenylalanine. Patients with phenylketonuria should be informed that Zolmitriptan orodispersible tablets contain phenylalanine (a component of aspartame).

Neither non-clinical nor clinical data are available to assess aspartame use in infants below 12 weeks of age.

4.5. Interaction with other medicinal products and other forms of interaction

Interaction studies were performed with caffeine, ergotamine, dihydroergotamine, paracetamol, metoclopramide, pizotifen, fluoxetine, rifampicin and propranolol and no clinically relevant differences in the pharmacokinetics of zolmitriptan or its active metabolite were observed.

Data from healthy subjects suggests there are no pharmacokinetic or clinically significant interactions between zolmitriptan and ergotamine. However, the increased risk of coronary vasospasm is a theoretical possibility, and concomitant administration is contraindicated. It is advised to wait at least 24 hours following the use of ergotamine containing preparations before administering zolmitriptan. Conversely it is advised to wait at least six hours following use of zolmitriptan before administering an ergotamine containing product (see section 4.3).

Following administration of moclobemide, a specific MAO-A inhibitor, there was a small increase (26%) in AUC for zolmitriptan and a 3 fold increase in AUC of the active metabolite. Therefore, a maximum intake of 5 mg zolmitriptan in 24 hours is recommended in patients taking a MAO-A inhibitor. The drugs should not be used together if doses of moclobemide higher than 150 mg b.i.d. are administered.

Following the administration of cimetidine, a general P450 inhibitor, the half life of zolmitriptan was increased by 44% and the AUC increased by 48%. In addition, the half life and AUC of the active, N-desmethylated, metabolite (N-desmethylzolmitriptan) were doubled. A maximum dose of 5 mg zolmitriptan in 24 hours is recommended in patients taking cimetidine. Based on the overall interaction profile, an interaction with specific inhibitors of CYP 1A2 cannot be excluded. Therefore, the same dosage reduction is recommended with compounds of this type, such as fluvoxamine and the quinolones (eg ciprofloxacin).

Selegiline (a MAO-B inhibitor) and fluoxetine (an SSRI) did not result in any pharmacokinetic interaction with zolmitriptan. However, there have been reports describing patients with symptoms compatible with serotonin syndrome (including altered mental status, autonomic instability and neuromuscular abnormalities) following the use of selective serotonin reuptake inhibitors (SSRIs) or serotonin norepinephrine reuptake inhibitors (SNRIs) and triptans (see section 4.4).

Undesirable effects may be more common during concomitant use of triptans and herbal preparations containing St John's wort (Hypericum perforatum).

As with other 5HT1B/1D receptor agonists, zolmitriptan could delay the absorption of other medicinal products.

Concomitant administration of other 5HT1B/1D agonists within 24 hours of zolmitriptan treatment should be avoided. Similarly, administration of zolmitriptan within 24 hours of the use of other 5HT1B/1D agonists should be avoided.

Interaction studies have only been performed in adults.

4.6. Fertility, pregnancy and breast-feeding

Pregnancy:

The safety of this medical product for use in human pregnancy has not been established. Evaluation of experimental animals studies does not indicate direct teratogenic effects. However, some findings in embryotoxicity studies suggested impaired embryo viability. Administration of zolmitriptan should only be considered if the expected benefit to the mother is greater than any possible risk to the foetus.

Breast-feeding:

Studies have shown that zolmitriptan passes into the milk of lactating animals. No data exist for passage of zolmitriptan into human breast milk. Therefore, caution should be exercised when administering zolmitriptan to women who are breast-feeding. Infant exposure should be minimised by avoiding breast-feeding for 24 hours after treatment.

4.7. Effects on ability to drive and use machines

Zolmitriptan Orodispersible Tablets has no or negligible influence on the ability to drive and use machines. In a small group of healthy individuals there was no significant impairment of performance of psychomotor tests with doses up to 20 mg zolmitriptan. Caution is recommended in patients performing skilled tasks (e.g. driving or operating machinery) as drowsiness and other symptoms may occur during a migraine attack.

4.8. Undesirable effects

Possible undesirable effects are typically transient, tend to occur within four hours of dosing, are no more frequent following repeated dosing and resolve spontaneously without additional treatment.

The following definitions apply to the incidence of the undesirable effects:

Very common ( ≥ 1/10); common ( ≥ 1/100 to < 1/10); uncommon ( ≥ 1/1,000 to < 1/100), rare ( ≥ 1/10,000 to < 1/1,000), very rare ( < 1/10,000).

Within each frequency grouping, undesirable effects are presented in order of decreasing seriousness.

4.9. Overdose

Volunteers receiving single oral doses of 50 mg commonly experienced sedation.

The elimination half-life of zolmitriptan tablets is 2.5 to 3 hours, (see section 5.2) and therefore monitoring of patients after overdose with Zolmitriptan Orodispersible Tablets should continue for at least 15 hours or while symptoms or signs persist.

There is no specific antidote to zolmitriptan. In cases of severe intoxication, intensive care procedures are recommended, including establishing and maintaining a patent airway, ensuring adequate oxygenation and ventilation, and monitoring and support of the cardiovascular system.

It is unknown what effect haemodialysis or peritoneal dialysis has on the serum concentrations of zolmitriptan.
