Uniroid-HC Ointment contains hydrocortisone and cinchocaine hydrochloride. Hydrocortisone soothes and relieves irritations and inflammation of the skin. Cinchocaine hydrochloride, a local anaesthetic, provides pain relief. It is used to treat: external haemorrhoids (piles), providing short-term relief of pain, irritation and associated itching around the back passage (anus) internal haemorrhoids using the nozzle applicator provided. 2.

Do NOT use Uniroid-HC Ointment if you: are allergic to hydrocortisone, or to any other steroids are allergic to cinchocaine, or to any other local anaesthetics are allergic to any of the other ingredients (listed in section 6) have tuberculosis have any kind of skin infection (e.g. thrush of the back passage, viruses affecting the skin). Do not use ointment in children under 12 years of age unless directed by the doctor. If any of the above apply to you, talk to your doctor or pharmacist. Warnings and precautions Talk to your doctor or pharmacist before using the ointment if you: have had a bad reaction to this or similar products before are pregnant or planning a pregnancy are breast-feeding. Contact your doctor if you experience blurred vision or other visual disturbances. Apply only to the back passage and the surrounding skin areas. Hydrocortisone can cause thinning and damage to the skin.

Do NOT cover the skin with bandages, plasters or any other dressing. However, it is safe to cover the skin with your clothes. Children and adolescents This medicine is NOT suitable for children under 12 years. Other medicines and Uniroid-HC Ointment Please tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including medicines obtained without a prescription, and herbal preparations. Some medicines may be affected by Uniroid-HC or they may affect how well Uniroid-HC will work. Some medicines may increase the effects of Uniroid-HC Ointment and your doctor may wish to monitor you carefully if you are taking these medicines (including some medicines for HIV containing cobicistat). Pregnancy and breast-feeding Do not use the ointment if pregnant or breast-feeding unless advised by your doctor. Ask your doctor or pharmacist for advice before taking any medicine if you might be pregnant or breast-feeding. Uniroid-HC Ointment contains cetostearyl alcohol Uniroid-HC Ointment contains cetostearyl alcohol. This may cause local skin reactions, such as contact dermatitis. 3.
Uniroid-HC Ointment
Always use Uniroid-HC Ointment exactly as your doctor has told you. Check with your doctor or pharmacist if you are not sure. Using this medicine To open the tube remove and turn the cap upside down and use it to pierce the seal at the top of the nozzle. Gently wash the area around the back passage with water and pat dry with cotton wool. Using without the applicator 1. Wash your hands, then squeeze a small quantity (a pea-sized amount) of the ointment onto a finger and spread it onto the affected area without rubbing. Do NOT use toilet paper to apply the ointment. 2. Wash your hands thoroughly. Using with the applicator (inside the back passage) 1. Your ointment contains a plastic nozzle applicator to help you apply the ointment inside the back passage. Clean the nozzle applicator thoroughly in warm, soapy water before and after each use. 2. Screw the nozzle applicator onto the end of the ointment tube and insert the nozzle applicator fully into the back passage. 3. Squeeze the tube gently from the lower end while slowly pulling it out of the back passage. 4. Stay still for 1 or 2 minutes. 5. Unscrew the nozzle applicator and wash both it and your hands. Do NOT apply a dressing after using this ointment as this can increase the risk of an allergic reaction (see section 4). Doses Adults, elderly and children over 12 years: apply the ointment twice a day (morning and evening) and after each time you empty your bowels unless your doctor has told you otherwise.

Do not use the ointment for more than 7 days. If the problem is no better, tell your doctor. Children under 12 years: this medicine is NOT suitable for children under 12 years. If you use more Uniroid-HC Ointment than you should If you apply too much ointment, wipe off the excess with cotton wool. If you accidentally insert too much ointment, tell your doctor or pharmacist. If you or anyone else swallows any of the ointment: 1. Tell your doctor, pharmacist or nearest hospital casualty department immediately. 2. Take the ointment tube with you so that people can see what you have used or taken. 3. Do this even if you feel well. If you forget to use Uniroid-HC Ointment If you forget to use the ointment, use it as soon as you remember, but if it is almost time for your next dose, skip the missed dose and continue as usual. Do not use a double dose to make up for a forgotten dose. If you have any further questions on the use of this product, ask your doctor or pharmacist. 4.
Like all medicines, Uniroid-HC Ointment can cause side effects, although not everybody gets them. Tell your doctor if you get the following side effect: blurred vision or other visual disturbances an allergic reaction such as itching, pain or rash around the back passage thinning of the skin. Large doses and regular use may suppress the adrenal glands. This may cause complex changes in your body. If you use Uniroid-HC Ointment regularly, discuss this with your doctor. Reporting of side effects If you get any side effects, talk to your doctor, pharmacist or nurse. This includes any possible side effects not listed in this leaflet. Uniroid-HC Ointment
Keep this medicine out of the sight and reach of children. Do not use after the expiry date stated on the tube and on the carton after EXP. The expiry date refers to the last day of that month. Do not store above 25°C. Store in the original package. Do not throw away any medicines via wastewater or household waste. Ask your pharmacist how to throw away medicines you no longer use. These measures will help to protect the environment. 6.

Uniroid-HC Ointment
Each gram of ointment contains
hydrocortisone 5 mg and cinchocaine hydrochloride 5 mg

Ointment.
An off-white, odourless, smooth, translucent ointment.
Uniroid-HC Ointment is indicated primarily for the treatment of external haemorrhoids for the short term relief of pain, irritation and associated pruritus ani. Uniroid-HC Ointment can also be used for internal haemorrhoids.
Posology
Adults
Treatment with Uniroid-HC Ointment should be limited to seven days. Patients should be advised to return to their doctor if the condition persists beyond this time.
Directions for use and dosage schedule:
First wash the anal area gently with water and pat dry with cotton wool. With the finger, spread a small quantity of the ointment on the painful area without rubbing. Do not use toilet paper.
Apply the ointment twice a day (morning and evening) and after each bowel movement, or as prescribed by the doctor.
The ointment can be used internally by means of the nozzle applicator which is supplied. Insert the nozzle applicator to full extent and squeeze the tube gently from the lower end whilst withdrawing.
The nozzle applicator must be cleaned thoroughly in warm, soapy water before and after each use.
The ointment may be used separately or concurrently with the suppositories.
Elderly
Dosage modifications are not required in the elderly.
Paediatric population
Uniroid-HC Ointment is not recommended for use in children under 12 years of age unless directed by a doctor.
Method of administration
For rectal use only
Hypersensitivity to the active substances or to any of the excipients listed in section 6.1.
This product is contraindicated in tuberculosis, anal thrush and most viral lesions of the skin including herpes simplex, vaccinia and varicella.
Apply only to the region of the rectum and anus and surrounding skin. Hydrocortisone can cause thinning and damage to the skin.
Recurrent or prolonged application may increase the risk of contact sensitisation particularly to cinchocaine.
As with all preparations containing topical steroids, the possibility of systemic absorption should be considered. In particular, long-term continuous therapy should be avoided in infants. Adrenal suppression can occur even without occlusion.
Visual disturbance may be reported with systemic and topical corticosteroid use. If a patient presents with symptoms such as blurred vision or other visual disturbances, the patient should be considered for referral to an ophthalmologist for evaluation of possible causes which may include cataract, glaucoma or rare diseases such as central serous chorioretinopathy (CSCR).
Excipients
This medicinal product contains cetostearyl alcohol. This may cause local skin reactions, such as contact dermatitis.
Co-treatment with CYP3A inhibitors, including cobicistat-containing medicinal products, is expected to increase the risk of systemic side-effects.
The combination should be avoided unless the benefit outweighs the increased risk of systemic corticosteroid side-effects, in which case patients should be monitored for systemic corticosteroid side-effects.
Pregnancy
In pregnant animals, administration of corticosteroids can cause abnormalities of foetal development. The relevance of this finding to human beings has not been established. However, topical steroids should not be used extensively in pregnancy, i.e. in large amounts or for long periods.
Breastfeeding
Hydrocortisone may pass into human breast milk. Given the possible maternal systemic absorption and lack of data, Uniroid-HC Ointment should preferably not be used during lactation unless the potential benefits to the mother outweigh the potential risks, including those to the breastfed child.
Uniroid-HC Ointment has no or negligible influence on the ability to drive and use machines.
The frequencies of adverse events are ranked according to the following convention: very common (≥1/10), common (≥1/100 to <1/10), uncommon (≥1/1,000 to <1/100), rare (≥1/10,000 to <1/1,000), very rare (<1/10,000), not known (cannot be estimated from the available data).
Endocrine disorders
Not known: Adrenal suppression (see section 4.4).
When applied topically and to a large enough area, especially of damaged skin for long enough, or if under occlusive dressing, hydrocortisone may have this adverse effect.
Eye disorders
Not known: Chorioretinopathy, vision blurred (see section 4.4).
Skin and subcutaneous disorders
Not known: Urticaria, rash (see section 4.4).

Reporting suspected adverse reactions after authorisation of the medicinal product is important. It allows continued monitoring of the benefit/risk balance of the medicinal product. Hydrocortisone
The principle pharmacological actions of hydrocortisone are on gluconeogensis, glycogen deposition, protein and calcium metabolism and inhibition of corticotrophin secretion and anti-inflammatory activity (glucocorticoid actions). When applied topically hydrocortisone causes reduction of inflammation, pruritus and exudation in disorders of the skin and perianal region.
Cinchocaine hydrochloride
Cinchocaine hydrochloride is a local anaesthetic agent and is suitable for surface or spinal anaesthesia and for relaxing sphincteric spasms. It is an anaesthetic of the amide type. It is more toxic than cocaine by local application, but its local anaesthetic action is greater, so it can be used in lower concentrations. Its action is more prolonged than lignocaine.
Surface or topical anaesthetics such as cinchocaine block the sensory nerve endings in the skin preventing transmissions of impulses along the nerve fibres and inhibiting depolarisation and ionexchange. These effects are reversible. Before this blocking action can occur the lipid, soluble anaesthetic base must penetrate the lipoprotein nerve sheath and the effectiveness of the anaesthetic depends on the concentration attained in the nerve fibre. The onset of action varies depending on the anaesthetic used. Cinchocaine has a rapid onset of action and is also long lasting.
Hydrocortisone
Hydrocortisone is passed through the skin, particularly in denuded areas. About 90% of plasma hydrocortisone is bound to plasma proteins, mainly to globulin, less so to albumin. In the liver and most body tissues it is metabolised to hydrogenated and degraded forms such as tetrahydrocortisone and tetrahydrocortisol. These degraded forms are excreted in the urine. They are mainly conjugated as glucuronides. A very small proportion of unchanged hydrocortisone is excreted in the urine.
Cinchocaine hydrochloride
Most local anaesthetics such as cinchocaine hydrochloride are absorbed through damaged skin. Cinchocaine hydrochloride is an ester-type local anaesthetic. Following absorption, it is hydrolysed by esterases in the plasma and liver.
There are no pre-clinical data of relevance to the prescriber which are additional to that already included in other sections of the SPC.
