What it is and what it is used for

for Cholurso contains the active substance ursodeoxycholic acid. Ursodeoxycholic acid is a chemical present naturally in the body and it helps to control the amount of cholesterol in the blood. Cholurso is used

  • to dissolve gallstones caused by excess cholesterol in the gall bladder (in patients for whom surgery is not an option), where the gallstones are not visible on a plain x-ray (gallstones that are visible will not dissolve) and not more than 15 mm in diameter. The gall bladder should still be working despite the gallstone(s).
  • for the treatment of a condition where the bile ducts in the liver become damaged leading to a buildup of bile. This may cause scarring of the liver. The liver should not be so damaged that it is not functioning properly. This condition is called primary biliary cholangitis.
  • for the treatment of liver disease associated with a condition called cystic fibrosis in children and adolescents aged 6 to 18 years.

What you need to know before you take it

e Cholurso 250 mg or 500 mg film-coated tablets Do not take Cholurso:

  • if you are allergic to bile acids like ursodeoxycholic acid or any of the other ingredients of this medicine (listed in section 6);
  • if you are allergic to peanut or soya, because Cholurso contains soya lecithin;
  • if your gall bladder does not work properly;
  • if you have gallstones that are visible on an x-ray;
  • if you have acute inflammation of the gall bladder or biliary tract;
  • if you have an obstruction of the biliary tract (blockage of the common bile duct or a cystic duct);
  • if you suffer from frequent cramp-like pains in the upper abdomen (biliary colic);
  • if you have hardening of the gallstones caused by a build-up of calcium;
  • if you are a child with biliary atresia and have poor bile flow, even after surgery. Ask your doctor about any of the conditions mentioned above. You should also ask if you have previously had any of these conditions or if you are unsure whether you have any of them. Warning and precautions Talk to your doctor, pharmacist or nurse before taking Cholurso. Cholurso must be used under medical supervision. Your doctor should test your liver function regularly every 4 weeks for the first 3 months of treatment. After this time, it should be monitored at 3 month intervals. If you are taking Cholurso for dissolution of gallstones, you should use an effective non-hormonal method of contraception, since hormonal contraceptives may increase biliary lithiasis (see "Other medicines and Cholurso" and "Pregnancy, breast-feeding and fertility"). Inform your doctor immediately if you have diarrhoea as this may require a reduction in the dose or stopping the treatment. Colestyramine, charcoal, colestipol (to lower blood lipids) or antacids containing aluminium hydroxide or smectite (aluminium oxide) should not be used simultaneously with Cholurso (see other medicines and Cholurso). Other medicines and Cholurso Tell your doctor, pharmacist or nurse if you are taking, have recently taken or might take any other medicines, even if they are obtainable without a prescription. Treatment with Cholurso may still be allowed. Your doctor will know what is right for you. A reduction in the effects of the following medicines is possible when taking Cholurso:
  • colestyramine, charcoal, colestipol (to lower blood lipids) or antacids containing aluminium hydroxide or smectite (aluminium oxide). If you must take medicines that contain any of these ingredients, you should take these at least two hours before or after Cholurso.
  • ciprofloxacin and dapsone (antibiotics), nitrendipine (used to treat high blood pressure). It may be necessary for your doctor to change the dose of these medicines. An increase in the effects of the following medicines is possible when taking Cholurso:
  • ciclosporin (to reduce the activity of the immune system). If you are being treated with ciclosporin, your doctor should check the amount of ciclosporin in your blood. Your doctor will adjust its dose, if necessary. Inform your doctor if you are taking any blood cholesterol lowering agents such as clofibrate or medicines that contain oestrogen (estrogen), especially if you are taking Cholurso for the dissolution of gallstones, as they may stimulate the formation of gallstones. Pregnancy, breast-feeding and fertility If you are pregnant or breast-feeding, think you may be pregnant or are planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.
  • You should not take this medicine during pregnancy unless your doctor thinks it is absolutely necessary. Use of Cholurso during the first three months of pregnancy may affect the foetus.
  • Check that you are not pregnant before taking this medicine.
  • Use a reliable method of contraception – non-hormonal contraceptives (barrier methods) or lowoestrogen oral contraceptives are recommended. If you are taking this medicine for dissolution of gallstones, only use non-hormonal contraceptives, as hormonal oral contraceptives may stimulate the formation of gallstones.
  • Do not take this medicine if you are breast-feeding as the active ingredient in this medicine may pass into breast milk. If treatment with Cholurso is necessary, then stop breast-feeding.
  • The data available so far does not suggest any effect on fertility from this treatment. Driving and using machines

No particular precautions are necessary. Important information about some of the ingredients of Cholurso Cholurso contains soya lecithin. If you are allergic to peanut or soya, do not use this medicinal product. This medicine contains less than 1 mmol sodium (23 mg) per tablet, that is to say essentially 'sodiumfree'.

How to take it

Cholurso 250 mg or 500 mg film-coated tablets Always take this medicine exactly as your doctor or pharmacist has told you. Check with your doctor or pharmacist if you are not sure. The tablet of Cholurso 500 mg can be divided into equal doses. Swallow the tablets with a drink of water or other liquid. Do not crush or chew the tablets. Take the tablets regularly. The daily dose below serves as an example of how you can take the tablets. Use in patients with damage to liver tissue by impaired bile flow (Primary Biliary Cholangitis) Dosage For Cholurso 250 mg film-coated tablets Your doctor will determine your dose depending on your body weight. During the first 3 months of treatment, you should take 3-7 tablets in divided doses throughout the day at mealtime. As the liver function improves, the total daily dose can be taken once, in the evening.

Duration of treatment Your doctor will determine the duration of treatment. It may be necessary to get your blood checked regularly by the laboratory. Cholurso may be continued indefinitely in cases of primary biliary cholangitis. Note: In patients with primary biliary cholangitis, the symptoms may worsen at the start of treatment, for example the itching may increase. This only occurs in rare cases. If this happens, therapy can be continued with a lower daily dose of Cholurso. Each week, your doctor will then gradually increase the daily dose, until you reach the required dose. Use in patients with gallstones Dosage The recommended dose is approximately 10 mg ursodeoxycholic acid per kg body weight per day, taken as follows:

Duration of treatment It generally takes 6-24 months to dissolve gallstones. The duration of your treatment depends on the size of the existing gallstones at the beginning of the treatment. Even if your symptoms have disappeared, you should continue with the treatment. Stopping treatment may result in the extension of

the total treatment time. After the gallstones have been dissolved, the treatment should be continued for 3-4 months. If there is no reduction in the size of the gallstones after 12 months, therapy should be stopped. Every 6 months, your doctor should check whether the treatment is working. At each of these followup examinations, it should be checked whether a build-up of calcium causing hardening of the stones has occurred since the last time. If this happens, your doctor will stop the treatment. If you feel that the effect of Cholurso is too strong or too weak, talk to your doctor or pharmacist. Use in children and adolescents (6 to 18 years) for the treatment of liver disease associated with cystic fibrosis Dosage The recommended daily dose is 20 mg/kg of body weight, divided into 2-3 doses. The administration of Cholurso film-coated tablets is based on body weight and the medical condition. If the patient cannot swallow tablets or has a bodyweight below 47 kg, other pharmaceutical forms with ursodeoxycholic acid (suspension) should be checked for availability. If you take more Cholurso than you should If you or anyone else take too many film-coated tablets, seek immediate medical attention. If you forget to take Cholurso Do not take a double dose to make up for a forgotten dose. If you stop taking Cholurso Always speak to your doctor before you decide to interrupt treatment with Cholurso or to stop your treatment early. If you have any further questions on the use of this medicine, ask your doctor, pharmacist or nurse.

Possible side effects

Like all medicines, this medicine can cause side effects, although not everybody gets them. Contact your doctor immediately if you experience any of the following symptoms after taking Cholurso: Common side effects (may affect up to 1 in 10 people):

  • soft, loose stools or diarrhoea. Inform your doctor immediately if you have persistent diarrhoea, as this may require a reduction in the dose of your medicine. If you do suffer from diarrhoea, make sure you drink enough liquids to replace your fluid and salt balance. Diarrhoea may also occur as a result of overdose. Very rare side effects (may affect up to 1 in 10,000 people):
  • during the treatment of primary biliary cholangitis: severe right-sided upper abdominal pain, severe worsening of liver scarring – this partially improves after treatment is stopped;
  • hardening of gallstones due to build up of calcium. There are no additional symptoms of this but it will show up in tests;
  • nettle rash (urticaria). Not known (frequency cannot be estimated from the available data):
  • vomiting;
  • in patients with cirrhosis, exacerbation (increase in severity) of pruritus (itching) at the beginning of the treatment cannot be completely ruled out;
  • in patients with stage IV primary biliary cholangitis, there may be an increase in the concentration of certain enzymes (alkaline phosphates, gamma-glutamyltransferase) and bilirubin;
  • Jaundice. Reporting of side effects If you get any side effects, talk to your doctor, pharmacist or nurse. This includes any possible side effects not listed in this leaflet. How to store it

Cholurso 250 mg or 500 mg film-coated tablets

Keep this medicine out of the sight and reach of children. Do not use this medicine after the expiry date which is stated on the blister label and carton after 'EXP'. The expiry date refers to the last day of that month. This medicinal product does not require any special storage conditions. Do not throw away any medicines via wastewater or household waste. Ask your pharmacist how to throw away medicines you no longer use. These measures will help protect the environment.

4.1. Therapeutic indications

- Treatment of primary biliary cholangitis (PBC, also known as primary biliary cirrhosis) in adult patients without decompensated cirrhosis.

- Dissolution of radiolucent cholesterol gallstones in patients for whom surgical treatment is not indicated. The gallstones must not produce any shadows on the radiograph, and should not be of a greater diameter than 15 mm, and the gall bladder, despite the gallstone(s), must be functioning.

Paediatric population

Hepatobiliar disorder associated with cystic fibrosis in children aged 6 years to less than 18 years.

4.2. Posology and method of administration

Oral route.

For patients weighing less than 47 kg or patients who are unable to swallow Cholurso 250 mg film-coated tablets, other formulations with ursodeoxycholic acid (e g oral suspension) should be checked for availability.

The following daily dose is recommended for the various indications:

For treatment of primary biliary cholangitis (PBC)

Stage I-III

The daily dose is dependent on body weight and ranges from 3 to 7 film-coated tablets (12-16 mg ursodeoxycholic acid per kg of body weight).

During the first 3 months of treatment, Cholurso 250 mg film-coated tablets should be taken at mealtime in divided doses throughout the day. If liver function improves, the total daily dose can be taken once daily in the evening.

Stage IV:

In combination with increased serum bilirubin levels (> 40 μg/L; conjugated), only half the normal dosage should initially be given (see dosage for stages I - III), (6 - 8 mg ursodeoxycholic acid per kg body weight per day, equivalent to about 2 to 3 Cholurso 250 mg film-coated tablets).

Thereafter, liver function should be closely monitored for several weeks (once every 2 weeks for 6 weeks). If there is no deterioration in liver function (AP, ALAT, ASAT, gamma-GT, bilirubin) and if no increased pruritus occurs, the dosage can be increased further to the usual level. However, liver function should again be closely monitored for several weeks. Once again, if there is no deterioration in liver function, the patient can be maintained at the normal dosage over the long term.

In rare cases the clinical symptoms may worsen at the beginning of treatment, e.g. increased itching. Should this occur, therapy should be continued with one 250 mg tablet daily, and the therapy gradually increased (increase of the daily dose weekly by one 250 mg tablet) until the dose indicated in the respective dosage regimen is reached again.

Patients with primary biliary cholangitis (stage IV) without increased serum bilirubin levels are allowed to receive the normal starting dose immediately (see dosage stages I - III).

However, close monitoring of liver function, as described above, is likewise applicable in such cases; treatment of primary biliary cholangitis will need to be regularly assessed on the basis of liver (laboratory) values and clinical findings.

The tablets should be swallowed whole with some liquid. Care should be taken to ensure that they are taken regularly.

Dissolution of Gallstones:

Adults: Approx. 10 mg ursodeoxycholic acid (UDCA) per kg body weight per day according to:

- up to 60 kg: 2 tablets

- 61-80 kg: 3 tablets

- 81-100 kg: 4 tablets

- above 100 kg: 5 tablets

The tablet should be swallowed whole with some liquid in the evening before bedtime. Care should be taken to ensure that they are taken regularly.

Based on experience to date, the duration of the dissolution process with Ursodeoxycholic acid is 6 months to 2 years, depending on the initial size of the stones. For a proper assessment of the therapeutic outcome, it is necessary, at the start of treatment, to accurately determine the size of the existing stones and subsequently to monitor them regularly, for example, every 3 to 4 months, via new X-rays and/or ultrasound scans.

It should also be observed during follow-up examinations whether in the meantime there has been any calcification of the stones. If this is the case, the treatment should be ended.

In patients whose stones have not decreased in size after six months of treatment at the dosage stated, it is recommended that the biliary lithogenic index be determined via duodenal samples. If the bile has an index of > 1.0, it is unlikely that a favourable result can be obtained and it is better to consider a different form of treatment for gallstones. Treatment must be continued for 3 to 4 months after ultrasound follow-up has confirmed complete dissolution of the gallstones. Discontinuation of treatment for 3-4 weeks leads to a return of bile supersaturation and prolongs the overall duration of therapy. Discontinuation of treatment upon dissolution of the gallstones may be followed by a relapse.

Older people: There is no evidence to suggest that any alteration in the adult dose is needed but the relevant precautions should be taken into account.

Paediatric population

Children with cystic fibrosis aged 6 years to less than 18 years:

20 mg/kg/day in 2-3 divided doses, with a further increase to 30 mg/kg/day if necessary.

4.3. Contraindications

Cholurso 250 mg film-coated tablets should not be used in patients with:

• acute inflammation of the gall bladder or biliary tract

• occlusion of the biliary tract (occlusion of the common bile duct or a cystic duct)

• frequent episodes of biliary colic

• radio-opaque calcified gallstones

• impaired contractility of the gall bladder

• hypersensitivity to active substance or hypersensitivity to peanut or soya or to any excipients listed in section 6.1

Paediatric population

Unsuccessful portoenterostomy or without recovery of good bile flow in children with biliary atresia.

4.4. Special warnings and precautions for use

Cholurso 250 mg film-coated tablets should be taken under medical supervision.

During the first 3 months of treatment, liver function parameters AST (SGOT), ALT (SGPT) and γ-GT should be monitored by the physician every 4 weeks, thereafter every 3 months. Apart from allowing for identification of responders and non-responders in patients being treated for primary biliary cholangitis, this monitoring would also enable early detection of potential hepatic deterioration, particularly in patients with advanced stage primary biliary cholangitis.

In patients treated with ursodeoxycholic acid, caution should be taken to maintaining a biliary flux.

When used for dissolution of cholesterol gallstones:

In order to assess therapeutic progress and for timely detection of any calcification of the gallstones, depending on stone size, the gall bladder should be visualised (oral cholecystography) with overview and occlusion views in standing and supine positions (ultrasound control) 6-10 months after the beginning of treatment.

If the gall bladder cannot be visualised on X-ray images, or in cases of calcified gallstones, impaired contractility of the gall bladder or frequent episodes of biliary colic, Cholurso 250 mg film-coated tablets should not be used.

Female patients taking Cholurso 250 mg film-coated tablets for dissolution of gallstones should use an effective non-hormonal method of contraception, since hormonal contraceptives may increase biliary lithiasis (see section 4.5. and 4.6.).

When used for treatment of advanced stage of primary biliary cholangitis:

In very rare cases decompensation of hepatic cirrhosis has been observed, which partially regressed after the treatment was discontinued.

In patients with PBC, in rare cases the clinical symptoms may worsen at the beginning of treatment, e.g. the itching may increase. In this case the dose of Cholurso should be reduced to one Cholurso 250 mg film-coated tablet daily and then gradually increased again as described in section 4.2.

If diarrhoea occurs, the dose must be reduced and in cases of persistent diarrhoea, the therapy should be discontinued.

Ursodeoxycholic acid must not be used at doses beyond 20 mg/kg/day regarding the potential higher risk of treatment failures found in patients with primary sclerosing cholangitis.

Cholurso 250 mg film-coated tablets should not be administered concomitantly with colestyramine, colestipol or antacids containing aluminium hydroxide and/or smectite (aluminium oxide) (see section 4.5).

Soya lecithin:

This medicine contains soya lecithin.

If a patient is hypersensitive to peanut or soya, Cholurso 250 mg film-coated tablets should not be used.

This medicine contains less than 1 mmol sodium (23 mg) per tablet, that is to say essentially 'sodium-free'.

4.5. Interaction with other medicinal products and other forms of interaction

Cholurso 250 mg film-coated tablets should not be administered simultaneously with colestyramine, colestipol or antacids containing aluminium hydroxide and/or smectite (aluminium oxide), because these preparations bind ursodeoxycholic acid in the intestine and thereby inhibit its absorption and efficacy. Should the use of a preparation containing one of these substances be necessary, it must be taken at least 2 hours before or after Cholurso 250 mg film-coated tablets (see section 4.4).

Cholurso 250 mg film-coated tablets can affect the absorption of ciclosporin from the intestine. In patients receiving ciclosporin treatment, blood concentrations of this substance should therefore be checked by the physician and the ciclosporin dose adjusted if necessary.

Due to the effect of ursodeoxycholic acid on the secretion of bile acids there is a theoretical possibility that the absorption of other lipophilic substances could be affected.

In isolated cases Cholurso 250 mg film-coated tablets can reduce the absorption of ciprofloxacin.

In a clinical study in healthy volunteers concomitant use of UDCA (500 mg/day) and rosuvastatin (20 mg/day) resulted in slightly elevated plasma levels of rosuvastatin. The clinical relevance of this interaction also with regard to other statins is unknown.

Ursodeoxycholic acid has been shown to reduce the plasma peak concentrations (Cmax) and the area under the curve (AUC) of the calcium antagonist nitrendipine in healthy volunteers. Close monitoring of the outcome of concurrent use of nitrendipine and ursodeoxycholic acid is recommended. An increase of the dose of nitrendipine may be necessary.

An interaction with a reduction of the therapeutic effect of dapsone was also reported.

These observations together with in vitro findings could indicate a potential for ursodeoxycholic acid to induce cytochrome P450 3A enzymes. Induction has, however, not been observed in a well-designed interaction study with budesonide, which is a known cytochrome P450 3A substrate.

Oestrogenic hormones and blood cholesterol lowering agents such as clofibrate increase hepatic cholesterol secretion and may therefore encourage biliary lithiasis, which is a counter –effect to ursodeoxycholic acid used for dissolution of gallstones.

4.6. Fertility, pregnancy and lactation

Animal studies did not show an influence of ursodeoxycholic acid on fertility (see section 5.3). Human data on fertility effects following treatment with ursodeoxycholic acid are not available.

There are no or limited amounts of data from the use of ursodeoxycholic acid in pregnant women. studies in animals have shown reproductive toxicity during the early phase of gestation (see section 5.3). Cholurso 250 mg film-coated tablets must not be used during pregnancy unless clearly necessary. Women of childbearing potential should be treated only if they are using reliable contraception; non-hormonal or low-oestrogen oral contraceptive measures are recommended. However, in patients taking Cholurso 250 mg film-coated tablets for dissolution of gallstones, effective non-hormonal contraception should be used, since hormonal oral contraceptives may increase biliary lithiasis.

The possibility of a pregnancy must be excluded before beginning treatment.

According to few documented cases of breastfeeding women milk levels of ursodeoxycholic acid are very low and probably no adverse reactions are to be expected in breastfed infants.

4.7. Effects on ability to drive and use machines

Cholurso 250 mg film-coated tablets has no or negligible influence on the ability to drive and use machines.

4.8. Undesirable effects

The evaluation of undesirable effects is based on the following frequency data:

Increase of the serologic levels of alkaline phosphatase, γ-GT and bilirubin (in patients with an advanced stage of PBC).

Skin and subcutaneous tissue disorders

Urticaria

An exacerbation of pruritus (upon the beginning of UDCA administration in patients with cirrhosis).

Reporting of suspected adverse reactions

Reporting suspected adverse reactions after authorisation of the medicinal product is important. It allows continued monitoring of the benefit/risk balance of the medicinal product. 4.9. Overdose

Diarrhoea may occur in cases of overdose. In general, other symptoms of overdose are unlikely because the absorption of ursodeoxycholic acid decreases with increasing dose and therefore more is excreted with the faeces.

No specific counter-measures are necessary and the consequences of diarrhoea should be treated symptomatically with restoration of fluid and electrolyte balance.

Additional information on special populations:

Long-term, high-dose ursodeoxycholic acid therapy (28-30 mg/kg/day) in patients with primary sclerosing cholangitis (off-label use) was associated with higher rates of serious adverse events.
